RED-first slice for #246 under #1.

Exact RED head 0812966b0d2b2edfa343cc17ea6f55daffd40d34 requires completed tasks to reject both repeated completion and later evidence mutation with task-already-completed, preserving terminal state and evidence.

No Core changes, cross-plugin private imports, arbitrary Prolog execution, or second runtime.
