Salvages the reusable, architecture-compatible part of #231 / #233 onto current main.

Kept

rlm_runtime_status as a domain-neutral library projection for:
- model identity;
- cumulative prompt/input tokens;
- cumulative completion/output tokens;
- explicit current context occupancy/window;
- derived context percentage when capacity is known;
- ctx ? when capacity is unknown.
deterministic coverage proving cumulative usage is not accepted as a substitute for current-context observation;
production-module load/readiness coverage.

Deliberately not carried forward

bundled DSH/reference-IDE ownership from old #231/#233;
rlm_cli_app;
prolog-rlm ide;
the old run wrapper/alias work;
DSH-specific launch contracts or semantic integration.

Current main now treats standalone AgentProlog as the downstream product and keeps prolog-rlm domain-neutral, so this PR only preserves the generic runtime contract useful to AgentProlog, DSH adapters, Emacs/JS/CL/Nim clients, or any other frontend.

Supersedes the reusable portion of #233. Related: #141, #184, #231.
