README task-state documentation is stale after the verified #242/#244/#246 slices. It still says completion only fails while no evidence exists, omitting typed passed/failed evidence, passing-verification requirement, and terminal completed-task behavior.

Acceptance:

document canonical evidence statuses
document verification-evidence-required, passing-verification-required, and task-already-completed
explicitly state completed tasks reject later evidence mutation
no new semantics beyond current tested implementation
full registry/source/install/Nix/package gate before merge
