Issue #24 bounded database-owned slice, forward-reconciled onto current master after sibling Hackpert #76 advanced the base.

Adds a pure bridge from one validated tool call/result pair to one operation-scoped operational-KB assertion. The candidate records capability, terminal status, call identity, exact call/result evidence IDs, expert/version, and provenance. Provider output remains in the execution graph rather than being copied into the KB. Long-term/global promotion remains explicit and separate.

No Hackpert expert/orchestration files are touched. This replacement PR exists to obtain exact-head CI for the combined current tree rather than stale-merging #74.
